In a move aimed at bolstering the quality of post-hospital discharge care, the New Jersey-based Atlantic Health System has announced a collaboration with Dimer Health, a startup focused on reducing hospital readmission rates by addressing clinical care gaps. This partnership comes as a response to the complexities surrounding patient transitions from hospital to home.

Steve Sheris, M.D., executive vice president and chief physician executive of Atlantic Health System, emphasized the need for continuous support during these transitions. He stated, ‘We want to invest in preserving health and wellness upstream away from the more traditional sites of care such as hospitals.’ This partnership aims to transform how patients are connected with their healthcare providers post-discharge.

Caroline Hodge, M.S., PA-C, M.B.A., founder of Dimer Health and a cancer survivor herself, shared her inspiration for creating a solution that addresses the gaps in post-discharge care. She mentioned, ‘We say that just like the patient goes from the emergency room to being admitted to the hospital, the hospital can then hand off to the transitionist, and we can care for the patient until we can hand them back off to their primary care doctor.’ This innovative approach ensures patients are not left unsupported during their healing journey.

The collaboration has already yielded impressive results, with reports of a 65% reduction in readmissions and a high Net Promoter Score of 95. Hodge highlighted the personalized care plans made possible through technology, which allows for proactive monitoring and follow-ups that can adapt to patient needs.

Moreover, as Dimer Health expands its reach beyond New Jersey, the emphasis on improving patient outcomes remains a priority. Sheris highlighted, ‘What we are trying to do is leverage those best outcomes, move them upstream in the care continuum, and make them continuous and durable.’ The integration of Dimer’s technology within Atlantic Health System is seen as a significant step forward in enhancing the quality of care provided to patients.
